Church Prayer Request Form

FAQs

A prayer request is a simple way to share your needs, concerns, or hopes with God and the parish community. Writing one helps you focus your heart on God’s care and invites others to join you in intercession, strengthening the bond of faith and support within the Church.

You may share as much detail as you are comfortable with, but it is usually best to keep it brief and respectful. Focus on what you need prayer for such as healing, guidance, strength, or peace, without including unnecessary private information that you would prefer not to be shared widely.

Yes. You can submit a prayer request on behalf of family members, friends, or even strangers. If possible, ask their permission first, especially if the situation is sensitive, and be gentle and clear in how you describe their need.

Once received, your intention is added to the prayer chains or prayer groups of the parish, and faithful members will lift your request up during Mass, personal prayer, or designated prayer meetings. Unless you ask to be contacted, your request is usually kept confidential and shared only for the purpose of prayer.

A thanksgiving note helps you slow down, recognize God’s blessings, and respond to His goodness with gratitude. It shifts your focus from problems to God’s faithfulness, deepening your relationship with Him and inviting His peace into your heart.

Yes. If you prefer your prayer request to remain private within the prayer ministry or pastoral team, you can usually indicate this in the form or by checking the checkbox  (“Please keep this confidential”).
